Form for Damaged Items Only
Items that you received damaged from Japan must follow these rules or the claim will not be accepted.  If accepted, insurance claim payments will be made at least 6 months after your claim. This is the Japanese Postal System's schedule.
 
1. Your claim must be made within 72 hours of you receiving your item.
2. Your item must be shown to a postal worker to make a claim.  You must receive claim forms or proof that the postal worker has seen your damaged item.
3. Please take pictures of your items. (digital pictures, scans are acceptable).  This will help expedite your claim.  If you do not have pictures it will slow down the process.
