I personally applied for this years ago and have kept it sealed in the envelope at my family home.  
I submitted it myself for PSA grading.  
As of this listing, there are only 3 instances of PSA 10, and it seems they are selling for a considerable amount on a certain auction site overseas.  
If desired, I can include the backing paper and the original envelope with personal information cut out, but please be aware that the envelope has some wear and should be considered an extra addition.
